Nutriment Dietitians will soon be offering Accredited Lifestyle Modification Programs in accordance with Queensland Health and Australian General Practice Network.

An accredited Lifestyle Modification Program (LMP) under the Australian Government Prevention of Type 2 Diabetes Program and was developed by the experts at Baker IDI Heart and Diabetes Institute. It is designed for patients who are at risk of developing Type 2 Diabetes as determined on the AUSDRISK score. For more information visit

The Lighten Up to a Healthy Lifestyle Program
A healthy lifestyle group program for adults who want to improve their health. Lighten Up was developed by and is funded by Queensland Health. It assists people to eat well and be active, manage stress and make long term behaviour changes to support a healthy lifestyle. For more information visit

LMP's are run in small groups of up to 15 participants and usually consist of 7 sessions over a six - eight month period.

The Programs are subsidised by the government with participants charged a patient contribution of $50.00.

Eligibility Criteria includes:

- Aged between 40-49 years (15-54 years for Aboriginal and/or Torres Strait Islander people)
- Be identified as 'high risk' based on the AUSDRISK Screening Tool
- Have had a diagnosis of Type 2 Diabetes excluded
- Have had a health assessment (items 701, 703, 705, 707 or 715 for Aboriginal or Torres Strait Islander people)
- Have a referral from your GP

Reset Your Life Program topics include:

  • The risks of diabetes and their relationship to lifestyle risk factors
  • The importance of regular diabetes screening
  • Nutrition advice and education
  • Physical activity advice
  • Stress management
  • Reducing cardiovascular disease
  • Strategies to support the adoption and maintenance of healthy lifestyle changes
  • Smoking cessation and alcohol reduction advice, if required,
  • Information about community resources to provide support in maintaining lifestyle change.
